If 7-OH, 7-hydroxy, or concentrated kratom use has become difficult to stop, professional addiction treatment can help you understand your symptoms and determine the right level of care.
DreamLife Recovery provides individualized substance abuse treatment at its residential campus in Donegal, Pennsylvania.
7-OH, or 7-hydroxymitragynine, deserves particular attention because concentrated products can produce stronger opioid-like effects than traditional kratom leaf. Current medical literature has documented tolerance, dependence, cravings, and opioid-like withdrawal in some people using concentrated 7-OH products.

What Is 7-OH?
7-OH stands for 7-hydroxymitragynine, an alkaloid associated with kratom.
It may also appear on products as:
- 7-hydroxy
- 7-HMG
- 7-hydroxymitragynine
- Enhanced kratom
- Concentrated kratom extracts
Traditional kratom contains multiple naturally occurring compounds, with mitragynine being the primary alkaloid. 7-OH is normally present only in small amounts.
However, newer products may contain much higher concentrations of 7-OH.
The FDA warns that concentrated or enhanced 7-OH products may carry risks including addiction, dependence, and withdrawal.
This does not mean all kratom products carry exactly the same level of risk, but concentrated 7-OH products should not be treated as equivalent to traditional kratom leaf.
Can 7-OH Cause Dependence?
Yes.
Repeated exposure to concentrated 7-OH can lead to tolerance and physical dependence in some people.
Signs that use may be becoming problematic include:
- Needing larger amounts over time
- Taking 7-OH throughout the day
- Experiencing withdrawal between doses
- Strong cravings
- Using mainly to avoid feeling sick
- Repeated unsuccessful attempts to stop
- Spending increasing amounts of money on 7-OH
- Continuing to use despite physical or emotional problems
- Hiding use from family or friends
A published clinical case involving concentrated 7-OH described escalating use, tolerance, withdrawal, and medically managed treatment. Read the clinical case report.
What Does 7-OH Withdrawal Feel Like?
Research specifically involving concentrated 7-OH is still developing, but withdrawal can resemble opioid withdrawal.
Reported symptoms may include:
- Anxiety
- Restlessness
- Nausea
- Diarrhea
- Abdominal cramps
- Sweating
- Chills
- Muscle aches
- Sleep problems
- Irritability
- Strong cravings
The severity of withdrawal depends on factors such as how much someone uses, how often they use it, how long they have been using it, and whether other substances are involved.
Do I Need Medical Detox for 7-OH?
Not everyone using 7-OH needs inpatient detox.
However, medical evaluation may be important when:
- You are using concentrated 7-OH daily
- Withdrawal becomes difficult to manage
- You have repeatedly tried and failed to stop
- You have a history of opioid dependence
- Alcohol, benzodiazepines, or other opioids are also involved
- You have significant medical or mental health conditions
DreamLife Recovery's detoxification program provides medical monitoring and support for people beginning recovery from substance dependence.
Treatment should be individualized because there is no single standardized withdrawal protocol that applies to every person using 7-OH.
When 7-OH Use Becomes an Emergency
Dependence and withdrawal are different from an overdose or poisoning emergency.
America's Poison Centers reported increasing numbers of 7-OH exposure cases, with symptoms including nausea, agitation, confusion, breathing problems, excessive sleepiness, seizures, dependence, and withdrawal.
If someone:
- Cannot be awakened
- Has stopped breathing
- Has very slow or abnormal breathing
- Has a seizure
- Loses consciousness
- Appears seriously confused or medically unstable
Call 911 immediately.
